Is 'Formulary' Lethal Injection Okay?

BACKGROUND: Lethal injection is commonly used in the U.S. In the past, a "3-drug cocktail" was injected. The first drug is a sedative/pain killer. The second relaxes the muscles. The third stops the heart. Major drug companies no longer sell drugs used for executions. Several states now use small companies to mix "formulary" drugs (not FDA approved). Given this:
